SEE WHAT INFORMATION WE NEED TO APPROVE YOUR LOVED ONE FOR ADMISSION
– Most recent History & Physical
– Provider notes
– Current medication list
– Hospital visits from the past year
– Copy of recent Maximus assessment (for Long Term MaineCare referrals only)
– Confirmation of Long Term MaineCare approval (for Long Term MaineCare referrals only)
– Any additional information that would be helpful to get a clear picture of the potential resident’s health status
STEP 4: RECEIVE APPROVAL FROM THE ADMISSIONS TEAM
WHAT TO EXPECT AFTER APPROVAL & WHY SOMEONE MAY NOT BE APPROVED
A common question we are asked is what would cause a potential resident to be denied? One of the most common reasons we may deny a potential resident is he or she receives a medication we cannot administer due to our level of care and regulations. We may also deny a potential resident because of behaviors that would put the resident or others in danger.
STEP 5: CONFIRM PAYOR SOURCE
LONG TERM MAINECARE
1. Complete the Long Term MaineCare Application. This application can be submitted with supporting documentation to the DHHS MaineCare office closest to you, which are all listed on the first page of the application.
2. After submitting the application, you will receive a follow up response from DHHS. They may request additional information to determine if the potential resident is eligible for Long Term MaineCare coverage. If you have any questions regarding your Long Term MaineCare application or the status of your application, you can contact DHHS at 1-855-797-4357.
3. Once the Long Term MaineCare application has been approved, the potential resident will be placed in an approved pending placement status for Long Term MaineCare. He or she will remain in this status until he or she moves into a Long-Term Care Facility. When you and/or the potential resident are informed of this status change, let us know – this is when we can schedule the admission date (STEP 6!)
4. While working through the Long Term MaineCare application process, the potential resident will need to have a Maximus Assessment completed to determine which level of care he or she requires. To request this Assessment, you can complete the request form found in here on Maximus’ website. The completed form can then be submitted to Maximus via email at [email protected].
PRIVATE PAY
1. Complete Income Verification Form, provided by our Corporate Office. The Income Verification Form will be reviewed and we will determine if there is enough funding to move forward with the private pay admission, or if applying for Long Term MaineCare would be a better option.
2. The Income Verification Form does not allow our Company to request statements from the potential resident’s financial institution, therefore including copies of these statements is helpful in ensuring verification is completed quickly!
3. Once the income is verified and the potential resident is approved financially, our Corporate Office will draw up a bill to be paid on or before the date of admission. This bill includes a room deposit (equal to one month’s Room & Board) and first month’s payment to secure the room.
STEP 6: SCHEDULE ADMISSION DATE & WELCOME HOME!

– Clothing, including 5 or 6 outfits, 5 sets of undergarments, 4 nightgowns or pajamas, 1 coat, 2 pair of shoes, 2 pair of slippers, etc.
– Personal care items, including toothbrush, toothpaste, hair care items, etc.
– Electronics, including 32″ or smaller TV, laptop, tablet, cell phone, etc.
– Other belongings, including photos, decorations, recliner, etc.
